1. Usually one runs Detox Maintenance (DM) for the duration of running killing presets. This means you could be running DM a long time, for days or weeks, depending on how long you treat yourself.
2. I ran it non-stop for a year and a half.
3. I leave mine running continuously apart from when I am scanning. When I am running a scan on kill then I definitely run detox maintenance on remote.
