Is there a protocol that can help expel a sea urchin spike embedded in the foot for 10 months?

1. Organic foreign material will naturally react with the body's immune system and create an infection. This will react in such a way to try and expel the spike. You need to seriously look at getting it out first and then deal with the infection.
2. You have to try and dig it out using a needle or something sharp, even if the skin has closed up over it. I've been a surfer in Hawaii my entire life and can't count the number of times I've had to remove Wana (pronounced Va-na) from my feet due to walking over the reef. It's not going to dissolve or go away by itself. If you absolutely cannot find a way to dig it out or remove it, use something very acidic like vinegar and soak that part of the foot in it. This may help to dissolve it and neutralize some of the toxins depending on how deep it is but your best bet is to do anything possible to dig it out.
