Can I use the empty shell preset with MW setting with the cold lasers or must I stay with the cold laser settings? For what diseases would I need to put the cold laser in my ears or up my nose?

Use Presets->Shell (Empty) Preset->Spooky Cold Laser - JW
If you mean to run a Molecular Weight (MW) program with Cold Laser just load a MW program. If the MW program doesn't exist in the Spooky Database you can always create your own program if you know the MW of the substance you want.

You can use cold laser twin for Sinusitis, Tinnitus.
