1. If were me I would put the top of foot and back of the leg at the knee.
2. Creativity may be key. Try the end of the toe to top or bottom of the foot near the toe, or both sides of the toe. Hard to tell what would be best.
3. You can just put one pad below your big toe and the other pad on the top. I think it should work.
