If you are targeting a specific area, such as, kidney, would you place the tens pads one on the kidney and one on the abdomen? So frequencies would pass through the specific problem area?

This sounds reasonable to treat one kidney or the other. If you want to treat both, you might try placing the pads on each side just above the hip bones.
